The type of personal information I collect 

I may collect and process the following information:

· Name, Job Title and Contact details

· GP name and contact details

· Emergency/Next of Kin contact details

· Summary of our sessions


How I get the personal information and why I have it

Most of the personal information I process is provided to me directly by you for one of the following reasons:

· Internal record keeping

· To improve my services

· To send questionnaires or information you may find helpful

· In case of emergency


I may share this information if:

  • I believe you or someone you are in contact with is at risk of harm. I will endeavour to discuss this with you before hand
  • I am  legally obliged to do so - If a client discloses involvement in, or      information about acts of terrorism, the therapist is legally obliged to      inform the authorities, and cannot inform the client of their intention to      do so.
  • If a  client gives the therapist information regarding money laundering or      drug-trafficking offences, the therapist is obliged to pass this      information to the police.
  • A judge or coroner can legally order the release of client notes.
  • You request disclosure


Under the General Data Protection Regulation (GDPR), the lawful bases I rely on for processing this information are: 

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting CharlotteAOneill@protonmail.com

(b)  I have a contractual obligation. Where I am entering into a contract with you.

(c)  I have a legal obligation. Where I need to comply with a legal or regulatory  obligation, for example, where I might be ordered by a court to do so.

(d)  Vital interests.  Where it is necessary to protect the life or health (for example, in the case of medical emergency) or a safeguarding issue which requires me to share your information with emergency services.


How we store your personal information 

Your information is securely stored electronically, password protected and coded to protect your identification.

I will not keep your information for longer than is necessary and in line with my insurer’s requirements.  I will then dispose your information by deleting this from my hard drive and any other platform I may store this on. Any paper copies will be shredded and disposed of in a confidential manner.


Your data protection rights

Under data protection law, you have rights including:


Your right of access - You have the right to ask for copies of your personal information. 


Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete. 


Your right to erasure - You have the right to ask me to erase your personal information in certain circumstances. 


Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances. 


Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.


Your right to data portability - You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.


You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
